Oklahoma Library Association
Award Nomination Application 2007-2008

Criteria:

This award may be granted to a vendor who has longevity in participation at Oklahoma Library Association conferences, made significant contributions to the support of the association, and provided an outstanding level of service and support to Oklahoma libraries.

The Awards Committee will consider the length and breadth of service provided by an individual or company to the Oklahoma Library Association and Oklahoma libraries. This award may not be given every year.

If you prefer to submit a nomination by mail rather than using this online form, please print off and send in this form (PDF) as well as the three other documents listed below:

  • OLA Award Nomination Application 2007-2008
  • Please provide a letter of nomination that addresses the outstanding level of service to the Oklahoma Library Association and Oklahoma.
  • Please submit two letters of support.
