Please attach the following information:

1. Mission Statement:  Write one paragraph describing your view of technology's role
 in education.

2. Write an essay of 250 words on your philosophy of the integration of technology
 across the curriculum.

3. Include one lesson or unit integrating a cooperatively planned unit incorporating
 technology into the curriculum.

4. Include two letters of recommendation that reflect how technology has enhanced 
 teaching at your site.

Purpose:
1. To identify and reward library media specialists who practice integration of
 technology into the curriculum through collaborative planning and teaching in an
 exemplary manner.
2. To identify library media specialists who are involved in the promotion of the use
 of technology at all levels (teachers, administrators, and students) at their site
 and/or district.
3. To identify innovative visions of technology use in the educational setting.

Guidelines: 
1. The OASLMS Technology Award shall be presented to a building-level school
 library media specialist employed in the school during the year covered by the
 nomination.

2. The OASLMS Technology Award winner shall be a library media specialist who is
 a current member of the Oklahoma Association of School Library Media
 Specialists.

3. Nominations are to be made by school library media specialists, administrators,
 library supervisors, classroom teachers, or may be self nominated. Nominations
 must be made by a person employed in the school district during the year covered
 by the nomination.

4. Nominations can be made by requesting an application in writing from the State
 Department of Education, Library Media Section, 2500 N. Lincoln Boulevard,
 Oklahoma City, Ok 73105-4599 or from Kay Boies, Executive Director, OLA,
 300 Hardy Drive, Edmond, OK 73013. Applications can also be requested by
 telephone - (405) 521-2956 or OLA, (405) 348-0506. Applications can also be
 retrieved off the State Department web site located at sde.state.ok.us

5. The application should be completed by the nominee. The original and five copies
 of the application and all attachments should be returned to the State Department
 of Education, at the above address postmarked by February 1.

6. Readers shall be comprised of a five-member committee. The chairperson of
 OASLMS shall assemble the committee. The committee members are as follows:
 one district library and/or technology director, one current officer of OASLMS,
 one classroom teacher that is involved with technology, one former Technology in
 Education Award winner, (the first year this juror will be a former Polly Clarke
 Award Winner) and one library educator. No committee member may juror a
 library media specialist from his/her school district. 

7. The winner of the OASLMS Technology Award will be presented a plaque and
 cash award to be donated by a library-related company. The cash award should be
 applied toward the further development of the recipients skills related to
 technology in education.

8. The name of the winner of the OASLMS Technology Award will be announced at
 OLA each year. The recipient will be recognized again at Encyclomedia. Winners
 will be responsible for presenting a technology related session the following year at
 Encyclomedia and/or OLA.

Criteria for Judging:

Since this award is to recognize excellence in cooperative program planning and
integration of technology into the curriculum these factors may be considered:

1. Evidence of innovative use of technology that supports and extends the
 curriculum.

2. Evidence of cooperative program planning in the form of an instructional unit
 submitted with the application.

3. Evidence of professional involvement in both library media and instructional
 technology fields.

4. Evidence of professional leadership in the area of technology staff development.
